This Adoption Agreement may only be used in conjunction with the Plan document. All selections in the Adoption Agreement providing for
customized or “other” plan provisions are subject to review for administrative feasibility, and may be subject to additional fees.
Terms used in this Adoption Agreement which are defined in the Plan document shall have the meaning given them therein.
The Employer hereby acknowledges that it is adopting this Nonqualified Supplemental Deferred Compensation Plan. Federal legislation or other
changes in the law relating to nonqualified deferred compensation or other employee benefit plans may require that the Plan be amended.
