DELIVERY OF DOCUMENTS TO SHAREHOLDERS SHARING AN ADDRESS
In instances in which multiple holders of the Common Stock share a common address and are the beneficial
owners, but not the record holders, of those shares of Common Stock, the holders’ banks, brokers or other nominees may
only deliver one copy of this Proxy Statement and the Company’s 2015 Annual Report to Shareholders, unless the
applicable bank, broker or nominee has received contrary instructions from one or more of the shareholders. The
Company will deliver promptly, upon written request, a separate copy of this Proxy Statement and the Company’s 2015
Annual Report to Shareholders to any shareholder at a shared address to which a single copy of the documents was
delivered. A shareholder who wishes to receive a separate copy of this Proxy Statement and the Company’s 2015 Annual
Report to Shareholders should submit a request in writing to Robert J. Simmons, Chief Financial Officer of the
Company, 444 South River Road, St. George, Utah 84790, Telephone: (435) 634-3200. Beneficial owners sharing an
address who are receiving multiple copies of proxy materials and annual reports and who wish to receive a single copy of
such materials in the future will need to contact their broker, bank or other nominee to request that only a single copy of
each document be mailed to all shareholders at the shared address in the future.
OTHER BUSINESS
The Company’s management does not know of any other matter to be presented for action at the Meeting.
However, if any other matters should be properly presented at the Meeting, it is the intention of the persons named in the
accompanying proxy to vote said proxy in accordance with their best judgment.
