Form of RSU Agreement
Unless the Participant notifies the Company within ten (10) days following receipt of this Agreement that he or
she declines this Award, the Participant will be deemed to have accepted and agreed to the terms and conditions of this
Agreement and the Plan. The Participant acknowledges receipt of a copy of the Plan and represents that he or she is
familiar with the terms and provisions thereof, which are incorporated herein by reference.
